Additional Program Requirements
After receiving the conditional acceptance letter, students are required to complete and submit the additional requirements described below prior to beginning the program. Failure to meet, complete, and/or submit any of the below requirements may result in revocation of the student鈥檚 admission to the program.
Technical Standards (completed via ATCAS)
- Students are required to meet the technical standards of the program with or without accommodations.
- Students are required to complete and submit the Technical Standards Policy / Form.
Vaccination Records and Other Related Information (completed via ATCAS)
- Students are required to meet the program requirements for vaccinations and other required screenings (see Clinical Experience Requirements in Program Handbook).
- Students are required to submit vaccination records, results of a recent tuberculosis skin test, and a signed copy of the Hepatitis B Vaccination Policy / Form.
Communicable Disease Policy (completed via ATCAS)
- Students are required to review the Communicable Disease Policy and submit a signed copy of the Communicable Disease Policy / Form.
Bloodborne Pathogens Compliance Program (completed via ATCAS)
- Students are required to review the Bloodborne Pathogens Compliance Program and submit a signed copy of the Bloodborne Pathogens Policy / Form.
Confidentiality Agreement (completed via ATCAS)
- Students are required to review the Confidentiality Agreement Form and submit a signed copy of the agreement.
Release of Information Form (completed via ATCAS)
- Students are required to review the Release of Information Form and submit a signed copy of the form.
Criminal Background Check
- Students are required to complete a criminal background check through the Missouri Department Highway Patrol.
- Instructions for completing the criminal background check will be provided to each student via e-mail.
